Insufficiently Tightened Lug Nuts

If the lug nuts were not properly tightened, they can loosen, causing the to wheel separate from the vehicle, increasing the risk of a crash.

Summary

Isuzu Technical Center of America, Inc. (Isuzu) is recalling certain 2017-2020 NPR-HD, 2019-2020 NPR-XD, NQR, NRR, Chevrolet 4500HD, Chevrolet 5500HD and Chevrolet 5500XD, and 2020 Chevrolet 4500XD vehicles. The lug nuts on the rear outer wheels may not have been properly tightened when installed onto the rear axles.

 

Remedy

Isuzu will notify owners, and dealers will inspect the rear outer wheel lug nuts and tightening or replacing them as necessary, free of charge. The recall began December 20, 2019. Owners may contact Isuzu customer service at 1-866-441-9638 or Chevrolet customer service at 1-800-222-1020. Isuzu’s number for this recall is V1906.

INFORMATION

CONDITION

Isuzu Motors Limited has decided that a defect, which relates to motor vehicle safety, exists in certain 2017, 2019 and 2020 Isuzu N-Series vehicles equipped with diesel engines. In some of the affected vehicles, the rear outer wheel lug nuts may not have been properly tightened and/or torqued when installed onto the rear axles at the Baltimore port. The lug nuts may loosen and the wheel may ultimately separate from the vehicle, increasing the risk of a crash and/or injury to persons or property.

 

CORRECTION

Isuzu dealers are to inspect the rear outer wheels and lug nuts on both driver and passenger sides on affected vehicles and will properly tighten or install new lug nuts if necessary. This service will be performed free of charge.

 

VEHICLES INVOLVED

Involved are certain 2017, 2019 and 2020 Isuzu N-Series vehicles equipped with diesel engines.

Important: Dealers are to confirm vehicle eligibility prior to beginning repairs by using the Isuzu Vehicle Inquiry System (IVIS).

For dealers with involved vehicles, a report of involved vehicles containing the complete vehicle identification numbers has been or will be provided. Dealers will not have a report available if they have no involved vehicles currently assigned.

 

SERVICE PROCEDURE      

  1. Place the vehicle in park, apply the parking brake, and remove the key from the engine control switch.
  2. Inspect both driver side and passenger side rear outer wheels to ensure all lug nuts are present. Replace any missing lug nuts as necessary. Proceed to next step for proper torqueing procedure.
  3. Using an appropriate torque wrench, properly tighten all of the driver and passenger side rear outer wheel lug nuts to 500 Nm (362 lb ft) in the sequence set forth in Figure 1 below.

NOTE: Driver side rear outer wheel nuts are left-hand threaded and must be torqued counter-clockwise.

  1. Proceed to Applying the Campaign Label.

 

APPLYING THE CAMPAIGN LABEL

  1. Using a ball-point pen, fill in a campaign label (Part No. 2-90028-700-0) with Campaign Number V1906, Isuzu dealer code, and repair date.
  2. Affix the campaign label onto the driver’s side B-pillar.

DEALER RESPONSIBILITY

Dealers are to service all vehicles subject to this program at no charge to customers, regardless of mileage, age of the vehicle, or ownership. Customers who have recently purchased vehicles sold from your vehicle inventory, and for which there is no customer information indicated on the dealer listing, are to be contacted by the dealer. Arrangements are to be made to perform the required correction according to the instructions contained in this bulletin. Program followup cards should not be used for this purpose, since the customers may not as yet have received the notification letter.

In summary, whenever a vehicle subject to this program enters your vehicle inventory, or is in your dealership for service, you must take the steps necessary to ensure the recall remedy has been implemented before selling or releasing the vehicle.

Important:

As required by Federal law, dealers are not to deliver any new vehicle in their inventory, which is covered by a Safety Recall, until the necessary remedy has been performed.

To: Isuzu Dealer Principal, Sales Manager and Service Manager

Effective immediately, stop the sale and delivery of affected vehicles in your inventory, even if a customer has purchased an affected vehicle. We are in the process of taking steps to address a safety related defect in these vehicles. Federal law prohibits the sale and delivery to customers of any vehicle containing a safety defect. You must not deliver any covered vehicles to customers until after the vehicle has been remedied.

 

WHAT IS THE CONDITION?

Chronology of Defect / Noncompliance Determination

The N‐Diesel vehicles are shipped from Japan to the United States. The vehicles have two steer tires on the front axle and four drive tires on the rear axle. During transport, the rear outer wheels are mounted to the chassis of the vehicle.  In normal operation, the wheels are then installed on the rear axle once the truck is wholesaled and as it is otherwise being prepared for delivery to a bodybuilder or dealer. The rear wheel lug nuts are tightened, torqued and marked (with a marker). Before being released, the vehicles undergo a final quality control inspection.  Trucks that have not yet been wholesaled will receive periodic maintenance.  In April, 2019, due to high demand, staff at the Baltimore port began to install the rear wheels on some of the vehicles during maintenance rather than during the pre‐delivery process in order to reduce the burden on the installation staff.   In addition, temporary staff was added to the Baltimore port beginning in late April, 2019.

 

On October 15, 2019, Isuzu received a report indicating that 2 outer wheel lug nuts were found loose and were backing away from the wheel on a vehicle with about 100 miles of use.  Isuzu immediately reviewed warranty claims and identified three pertinent claims indicating that 1‐3 lug nuts were found to be missing.  The affected vehicles had repair order dates between June 11 and September 16, 2019, and mileages between 72‐8793 miles at the time of the repair. Each of the vehicles had been processed through the Baltimore port.  Two of the warranty claims involved vehicles that had been processed in early April, 2019.  While Isuzu will periodically receive isolated reports about lug nuts, the last such report or claim had been received in May, 2018, suggesting that this issue began in early April, 2019.

 

A containment hold was initiated at the Baltimore port on October 17, 2019.  That same day, Isuzu conducted an audit of 61 vehicles that were at the Baltimore port which were being prepared for shipping and found 17 with either loose or under‐torqued lug nuts.  Each of those 61 vehicles was reworked (every lug nut was torqued to spec) and released.  For all newly processed (or to be processed) vehicles the hold remained in place until an additional torque wrench arrived at the Baltimore port and new measures were implemented on October 22, 2019 to improve quality control.

 

Isuzu processed the issue through its internal investigation process, and on November 13, 2019 decided to conduct a safety recall in the field to inspect the outer rear wheel lug nuts on potentially affected vehicles processed through the Baltimore port starting between April 1, 2019 and October 17, 2019 and to tighten and properly torque the lug nuts if necessary.
